1. Information We Collect
We collect the following categories of personal information:
Identifiers: name, postal address, email address, telephone number, date of birth, and government-issued ID at the point of clinical service.
Commercial information: services purchased, treatment history, appointment records, and payment records.
Internet or network activity: IP address, browser type, device identifiers, pages viewed (including pages describing specific treatments and services), referring and exit pages, and timestamps collected automatically when you use the Site.
Geolocation data: approximate location derived from IP address.
Sensitive personal information: medical and health information you provide during intake, consultation, or treatment, including symptoms, medical history, lab results, medication use, and treatment plans.
Inferences: preferences and characteristics derived from the above, used to personalize service and follow-up communications.
2. Sources of Information
Directly from you when you complete intake forms, schedule appointments, communicate with us, or pay for services.
Automatically when you visit the Site, through cookies and similar technologies. • From our service providers, including our scheduling, payment, laboratory, and electronic health-record vendors.
3. How We Use Your Information
To provide medical evaluation, treatment, follow-up care, and related clinical services.
To schedule appointments, send appointment reminders, and respond to inquiries.
To process payments and maintain accurate financial and clinical records.
To operate, secure, and improve the Site and the Clinic's services.
To comply with applicable laws and respond to lawful requests.
With your separate written authorization, for any other purpose disclosed at the time of collection, including marketing.
4. How We Share Your Information
We share your information with the following categories of third parties, only as needed:
Healthcare service providers and laboratories involved in your treatment.
Website and operational service providers — including Squarespace (website hosting and forms), our scheduling system, our electronic health record or patient management system, our payment processor, and our email and text-message providers. These providers are contractually limited to processing data on our behalf for the purposes for which we engaged them.
Analytics providers — Google Analytics — that help us understand Site usage.
Advertising and measurement partners — including Google Ads and the Meta (Facebook) Pixel — currently across all pages of our Site, including pages describing specific treatments and services. These technologies may transmit information such as your IP address, device identifiers, browser cookie identifiers, and the URLs of pages you visit. We are actively working to restrict these advertising technologies to non-clinical pages of the Site; until that work is complete, you may opt out by the methods described in Section 6 below or by adjusting your browser settings as described in Section 8.
Regulators, law enforcement, or others when required by law or to protect rights, safety, or property.
We do not sell your medical information. Under CMIA, we will not disclose medical information for marketing or to third parties for their independent use without your prior written authorization.
5. How Long We Keep Your Information
Medical records: at least seven (7) years from the date of last service, consistent with California medical recordkeeping standards. Minor patients' records are retained for at least one (1) year after age of majority.
Billing and financial records: at least seven (7) years.
Website analytics and marketing data: up to twenty-six (26) months unless you opt out earlier.
Records of your privacy requests: at least twenty-four (24) months, as required by California law.
6. Your California Privacy Rights
If you are a California resident, you have the following rights regarding your personal information:
Right to Know: request the categories and specific pieces of personal information we have collected about you, the sources, the purposes, and the categories of third parties with whom we have shared it.
Right to Delete: request that we delete personal information we have collected from you, subject to legal exceptions (including our obligation to retain medical records).
Right to Correct: request correction of inaccurate personal information.
Right to Opt Out of Sale or Sharing: direct us not to "sell" or "share" your personal information for cross-context behavioral advertising. We do not sell personal information. To opt out of sharing for advertising purposes, email us at privacy@themarkmenshealth.com with the subject line "Opt Out" and we will process your request within fifteen (15) business days.
Right to Limit Use of Sensitive Personal Information: direct us to use your sensitive personal information only for the purposes necessary to provide our services. Email us at privacy@themarkmenshealth.com with the subject line "Limit Sensitive Information" to submit this request.
Right to Non-Discrimination: we will not deny services, charge different prices, or provide a different level of service because you exercise these rights.
Right to Authorized Agent: you may designate an authorized agent to submit a request on your behalf. We will verify the agent's authority.

We will respond within 45 days, with one 45-day extension permitted by law if needed. We may verify your identity before fulfilling a request.
7. Global Privacy Control
We are implementing automated honoring of the Global Privacy Control (GPC) browser signal as a valid opt-out of the sale or sharing of personal information. Until that implementation is complete, please email privacy@themarkmenshealth.com with the subject line "Opt Out" to submit your opt-out request, and we will process it within fifteen (15) business days. We anticipate completing automated GPC support in 2026.
8. Cookies and Tracking Technologies
We currently use cookies and similar tracking technologies across all pages of the Site, including:
Strictly necessary cookies required for the Site to function (provided by Squarespace).
Analytics cookies (Google Analytics 4) that help us understand how visitors use the Site.
Advertising cookies (Google Ads, Meta / Facebook Pixel) that help us measure and target advertising. These currently fire on every page of the Site, including pages describing specific treatments and services.
We are actively working to install a cookie consent banner that will allow you to accept or reject categories of cookies before they load. Until then, you may opt out of these technologies by:
Emailing privacy@themarkmenshealth.com with the subject line "Opt Out" — we will process your request within fifteen (15) business days.
Using your browser's privacy controls to block third-party cookies (Chrome, Safari, Firefox, and Edge all offer this option).
Installing a browser extension that blocks tracking pixels, such as Privacy Badger or uBlock Origin.
Opting out of Google Ads personalization at adssettings.google.com.
Opting out of Meta advertising in your Facebook account's Ad Preferences.
9. Children's Privacy
Our services are intended for adults. We do not knowingly collect personal information from anyone under the age of 18 through the Site. If we learn we have inadvertently collected personal information from a minor, we will delete it.
10. Shine the Light
California Civil Code §1798.83 permits California residents to request information about disclosure of personal information to third parties for those third parties' direct marketing purposes. 11. Data Security
We use administrative, technical, and physical safeguards designed to protect the personal information we maintain, including access controls, encryption in transit, and vendormanagement practices. No method of transmission or storage is perfectly secure, and we cannot guarantee absolute security.
12. Out-of-State Visitors
Our Site is operated from California. If you access the Site from outside California, your information will be transferred to and processed in California. We treat all visitors' information consistent with this Policy.
13. Changes to This Policy
We may update this Policy from time to time. We will post the updated version with a new "Last Updated" date. Material changes will be noticed conspicuously on the Site. This Policy will be updated when our cookie consent controls and automated Global Privacy Control honoring are deployed.
